BJC is looking for an RN Patient Care Supervisor for the hospice Central Referral Coordination team. The supervisor is responsible for educating hospital floor staff, physicians, social workers, palliative care, and case managers on the positive benefits hospice while supervising about 10 employees including registered nurses and social workers with a goal to encourage timely hospice referrals. Location and schedule Hospitals in St. Louis area including Barnes Jewish West County and St. Peters, Progress West, Christian, Barnes Jewish Hospital and Missouri Baptist Hospital. Mason Ridge Center Drive Home Office M-F 8a-4:30p - with admin on-call rotation Overview BJC Home Care offers patients and their families a complete range of home care services, including skilled nursing services, adult and pediatric hospice and supportive care, rehabilitation therapy, home infusion therapy, infusion treatment rooms, home medical equipment and high-tech respiratory care. Specialty home care programs also are available, including adult and pediatric asthma, cardiac, diabetes, orthopedic and wound care programs. BJC Home Care provides care to thousands of patients in both Missouri and Illinois. Serving more than 25 counties, it has become the largest home care network in the region and one of the largest in the country. The Central Referral Center (CRC) for BJC Home Care Services is where the patient referral process starts. There are Intake Coordinators in each BJC hospital who coordinate services for those patients that need home care. The patients who choose BJC Home Care Services are then channeled through the CRC. The CRC also consists of nurses who take physician office referrals for patients not currently hospitalized. There is someone on-call 24/7 for the CRC to facilitate home care coordination for patients in emergency rooms, etc. Preferred Qualifications Role Purpose Monitors and coordinates nursing and clinical services. Ensures services meet Medicare/Medicaid, TJC and department requirements. Responsibilities Manages individual(s) including but not limited to: hires, trains, assigns work, manages & evaluates performance, conducts professional development plans. Ensures that the productivity and actions of that group meet/support the overall operational goals of the department as established by department leadership. May participate in the development of departmental staffing, revenue and/or expense budgets and having direct responsibility for adhering to those goals. This includes responding to changes in the business which may affect the ability to achieve the budget goals. Manages and directs daily client care and staff activities. Performs on-site supervisory visits and serves as clinical resource to home care staff. Manages the hiring and performance management of staff. Coordinates services with other health care providers.
