Responsibilities The Case Manager provides services to patients and families acting as a liaison between treatment team, physician, referral source, community agencies, outside therapists, and third party reviewers. The Case Manager maintains the integrity of the medical record of assigned patients; assist in the preparation of the master treatment plan; initiates discharge planning; contacts discharged patients within established time frames for follow-up and assists in completing intake screenings and makes referrals. Performs other related duties as assigned by the Supervisor. Qualifications Bachelor's degree in social services field. Experience with children and adolescents. Background check to include FBI/BCI and drug screen required.
