Þtails Department: ACC Providers After Hours Schedule: Multiple Shifts: Full Time, Evening & Part Time, Nights & Days Every other Saturday & Sunday availability will be needed for all schedules. Responsibilities Care Navigators, for Providers after Hours, are supporing a 24/7 team assisting patients by phone in a remote setting. Care Navigators will receive high volume inbound calls to support callers to appropriate level of care, and as needed communicate with medical practices, providers and other general calls in multiple markets. Gather necessary demographic, and clinical information from patient and enters into appropriate database. Record and communicate patient's conditions as appropriate. Provides information, reference resources to support call resolution, paging and scheduling activity. Reschedules and cancels clinic appointments and provide general information leveraging EHR, a scheduling tool and resources for office practices. Direct callers to appropriate level of care. Seek appropriate resources to resolve issues about the type, date or location of prescribed procedures. Schedule patient procedures in a manner that most efficiently utilizes the patient's time and clinical resources. Coordinate and communicate schedules. Provide counseling to patient, participant or their representative regarding pre-service requirements and instructions. Demonstrates proficiency with on call applications, correctly identifying the call schedule and provider on call; uses provider preferences Demonstrates ability to work independently in a remote environment. Requirements Education: High School diploma equivalency OR 1 year of applicable cumulative job specific experience required. Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable. Additional Preferences Experience taking high volume calls in a health-care setting Ability to navigate multiple systems in a remote environment.
