
The APN Clinical Program Manager is responsible for coordination, and assurance of comprehensive health care using advanced practice nursing process and pertinent standards of care, the diagnosis of illness and physical conditions, and performance of diagnostic, therapeutic, corrective and prescriptive measures in collaboration with appropriate specialty and site based physicians. Serves as a liaison to nursing, physicians, department chairs/chiefs, care management, hospital administration, and other hospital departments. Adheres to programmatic compliance with regulatory agencies.
Clinical Practice
• Assesses the health status of patients/families by means of comprehensive health history and physical examination.
• Initiates selected therapeutic and diagnostic procedures, consultations and laboratory examination, prescribes medication regimens, interprets findings and alters plan of care according to accepted standards of care and practice guidelines
• Educates and counsels in the areas of health promotion, maintenance, disease prevention, and management of acute/chronic illnesses.
• Documents findings and plan of care in the patient's medical record.
• Consults with physician according to established standards, policies, procedures, and protocols.
• Initiates referrals to specialty services and fosters continuity of care.
• Participates in team meetings and conferences to enhance an interdisciplinary approach to care delivery.
• Performs a patient record review with the collaborating physician in a timely fashion but not less than every three months if utilizing a collaborative practice agreement.
• Practices as a member of the allied health staff according to the rules and regulations of the Medical Staff and the bylaws of the Hospital Staff.
• Maintains current expertise in area of practice
Administrative
• Collaborates with the Advanced Practice Nursing leads and Chair or Service Chief of the respective clinical department in the recruitment and retention of NPs.
• Guides and monitors performance and delivery of care on a daily basis.
• Facilitates the on-boarding and orientation as well as ongoing professional development of nurse practitioners.
• Coordinates clinical activities of nurse practitioners to accomplish established safety and quality measures.
• Provides leadership and develops effective team building.
• Sets programmatic goals in collaboration with division chief and other key personnel
• Demonstrates knowledge of and commitment to service excellence; role models Mount Sinai Health System standards.
• Participates in relevant hospital-wide committees.
• Acts as a liaison between inpatient and outpatient areas to assure continuity of care.
• Participates in performance appraisals of nurse practitioners on the assigned service.
• Collaborates with division chief and department administrators to ensure that systems supportive of clinical care function smoothly.
• Promotes utilization of NPs to operate at top of license.
Professional Practice
• Supports and role models the mission, philosophy, goals and objectives of the Department of Nursing and The Mount Sinai Health System.
• Promotes own professional growth and development in clinical and program managerial role.
• Participates in the development and monitoring of patient outcomes per established practice protocols for purposes of quality and performance improvement.
• Serves as a clinical role model and resource to the nursing and clinical staff.
• Serves as a professional resource to other members of the health care team.
• Uses evidence-based health care literature to advise and support appropriate practice changes within the designated service.
• Participates in Service Line Quality and Performance Improvement meetings.
• Participates in APN related meetings and committees.
