Assess patient needs regularly and assist in the developing and updating of the Master Treatment Plan. Provide clinically sound individual, family, and group therapies within required time frames and establish therapeutic relationships with patients and families. Work within a team environment to ensure that each treatment modality is appropriate for the patient. Provide treatment and case management activities to the patient, which may include individual and group therapy, linkage with community resources, and discharge planning. Perform social services functions to include assisting patients in meeting their biopsychosocial needs while enabling them to access through therapeutic treatment skills to achieve their optimal level of emotional health. Complete specialized clinical social work services for patients/families, crisis intervention, short-term and long-term family therapy, and provide referrals as needed. Complete and maintain all required documentation in a timely manner consistent with policies and regulations.
