Þscription Job Title: Social Worker – Women and Children's Health Social Work Services at the Mount Sinai Hospital The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities. The Bridge Clinical Program (3 DAYS PER WEEK) focuses on improving the health outcomes of women with substance use disorders. As the program is in early development, the role of the Social Worker in this new grant-funded position will be multifaceted and play a pivotal role in addressing substance use among a vulnerable population, fostering community relations, and providing direct support to a limited number of women, including those who are pregnant or parenting individuals who use drugs , as well as gynecological patients. This position requires a blend of advocacy, community development, clinical expertise, research, administrative skills, and ability to work independently. The program will be additionally supported by a peer recovery specialist who will collaborate closely with the social worker. Current funding for this position is for two years with the possibility of continuation through additional grants. Responsibilities The social worker in this position will provide inpatient and outpatient services with medical providers to ensure integrated care to a limited number of women, including pregnant and parenting individuals who use drugs. The social worker will be responsible for discharge planning at the time of delivery for postpartum patients with substance use disorders, coordinating with medical providers for a smooth transition to outpatient care. They will provide continuity of care from prenatal care through postpartum period, including evaluation for postpartum mental health disorders and recovery services. Individual therapy may extend to short-term mental health counseling and ancillary to the patient’s medical management including ongoing review of mental health with the program’s liaison psychiatrist. The social worker will conduct detailed alcohol and drug assessment to determine immediate and long-term service needs. The social worker will have programmatic development responsibilities within The Bridge Clinical Program as well. They will be responsible for establishing and maintaining strong relations with community organizations, stakeholders, and referral sources to ensure seamless services delivery and collaboration. The social worker will create/distribute education materials, manage social media, and design and conduct training programs for professionals working with pregnant and parenting individuals who use drugs. In their Float role (2 DAYS PER WEEK), the social worker will be responsible for providing coverage to social workers on various inpatient and outpatient Pediatric and OB service areas, but primarily in the NICU and OB. In this coverage role, the Social Worker will provide psychosocial support to children and families hospitalized or in ambulatory sub-specialty programs with acute and clinical illnesses, as well as assure timely, safe discharges. Coverage in the OB-GYN Associates clinical practice may include a variety of ambulatory and in-patient services, including planning for safe mother-baby discharge. The social worker collaborates with a large interdisciplinary group of providers and with many social work colleagues in Women and Children’s Health. This combined position is FULL-TIME with working days Monday to Friday, 9am – 5pm. The breakdown of days spent in each role may vary, but Wednesday is a required day in the Bridge Clinic. The position reports to Laudy Burgos, LCSW and program director, Dr. Habersham. Qualifications Excellent interpersonal, communication and organization skills. Ability to work independently and as part of a multidisciplinary team. Proficiency in using social medical platforms for outreach and advocacy. Experience, or significant interest, in policy advocacy, community organizing, public speaking and grant writing/management. Certified Alcoholism and Substance Abuse Counselor (CASAC) preferred. Bilingual (English/Spanish) preferred. LCSW preferred. Minimum of one year in current position unless currently a Float in Women?s and Children?s.
