Þscription Jon Title: Per Diem Social Worker II - Inpatient Med/Surg Social Work Services at the Mount Sinai Hospital The Mount Sinai Hospital is a 1,134-bed facility with an extensive outpatient and specialty care network and is the largest hospital in the Mount Sinai Health Care System. It offers comprehensive social work services and programming to meet the myriad needs of the diverse populations we serve. We believe that total patient care must emphasize the physical, emotional, and social needs of each patient and their care partners. Social Workers collaborate within interprofessional teams to serve patients and the larger community from both a micro and macro level including both direct care and prevention. Founded in 1907, the Department of Social Work Services at the Mount Sinai Hospital is one of the oldest hospital social work departments in the nation with over 450 licensed social workers employed across more than 65 different program/service areas. Employment in our department provides a pathway to LCSW licensure as well as access to a broad range of continuing education, professional development and extra-curricular opportunities. This position primarily involves the provision of psychosocial services to a variety of individuals with disabilities, including but not limited to patients who have recently experienced a spinal cord injury or traumatic brain injury. Responsibilities The social worker must coordinate complex discharges that requires extensive collaboration with the patient, identified caregivers, the inter-disciplinary care team and community agencies. The social worker provides ongoing support, advocacy and education to patients and families through individual sessions and family meetings. SW attends weekly inter-disciplinary team meetings. This is a temporary, per-diem position of between 4-6 months. The Inpatient Rehab at the Mount Sinai Hospital is expected to move to another location in January 2025 and thereafter through to approximately April 2024 the per diem social worker will cover Medicine beds. The ideal candidate would be available full-time, however, qualified applicants with part-time availability will be considered. Once the per diem position comes to an end, the person in this role would have the option to apply for other vacancies at Mount Sinai if they wish to do so. Qualifications NYS LMSW or LCSW. Inpatient hospital experience strongly preferred. Excellent organizational, collaborative, and psychosocial assessment skills. Knowledge of crisis intervention. Ability to work with multiple hospitals and community systems.
