Pharmacy Resident; Mount Sinai Queens; Full Time; Days

<br><b>Description</b><p>The Pharmacy Resident provides pharmaceutical care to unit specific patients including the establishment of therapeutic endpoints and monitoring parameters.</p><br><b>Responsibilities</b><p>1. Performs all duties associated with the Registered Pharmacist position within the scope of licensure.</p>
<p>2. Initiates and documents pharmacist interventions to change prescribing where required.</p>
<p>3. Participates in patient care rounds as appropriate.</p>
<p>4. Contributes to and participates in the departmental medication use evaluation plan, including but not limited to CMI (Clinical Management Initiatives).</p>
<p>5. Reviews current medical/pharmacy literature regularly for recent developments in disease state therapeutics as well as new product development.</p>
<p>6. Provides health professionals with drug information and/or references.</p>
<p>7. Prepares periodic information as newsletters for physicians, nurses and pharmacists regarding drug therapy and administration.</p>
<p>8. Provides inservice education seminars and programs to the pharmacy staff and other health care professionals.</p>
<p>9. Facilitates preparation of Pharmacy and Therapeutics Committee material.</p>
<p>10. Investigates and documents reports of Adverse Drug Reactions. Facilitates Adverse Drug Reaction Subcommittee meetings.</p>
<p>11. Completes all required documentation.</p>
<p>12. Completes a minimal equivalent of 47 weeks of active training to receive residency certificate.</p>
<p>13. Demonstrates the knowledge and skills necessary to provide care, based on physical,</p>
<p>14. Psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area.</p>
<p>15. Maintains patient/employee confidentiality in the management of information.</p>
<p>16. Observes the Health Care Systems compliance policies.</p>
<p>17. Acts as the managing pharmacist while working weekends.</p>
<p>18. Exercises sound judgement, recognizes and attends to priorities.</p>
<p>19. Participates in departmental quality assurance activities.</p>
<p>20. Meets regulatory, licensure and annual health assessment requirements.</p><br><b>Qualifications</b><p>Doctor of Pharmacy Degree from an accredited college of pharmacy.</p>
