<div class="mds-edited-text mds-font-body-copy-bulk"><p>Support Coordinator</p><p>This is a really exciting time to join the organisation as additional funding has meant the charity will be recruiting talented people to a number of new roles throughout the New Year.</p><p>There are the following Support Coordinator roles available;</p><p>Bradford &ndash; 1 x 35 hour post</p><p>Doncaster - 1 x 35 hour post</p><p><b>Position: </b>S992 Stroke Support Coordinator</p><p><b>Location:</b> Homebased, North East and Yorkshire, however, extensive travel will be required as part of this role (May include team meetings or other work related meetings)</p><p><b>Hours:</b> 35 hours per week &ndash; applications from people seeking flexible working, part-time and job shares are welcomed</p><p><b>Salary:</b> Circa &pound;24,370.00 per annum pro rata</p><p><b>Contract:</b> This is a fixed-term until 31 March 2024. Please let us know if this will present any challenges when you email your application.</p><p><b>The Role</b></p><p>This is an exciting opportunity to be involved in a brand new project in Yorkshire. The project is funded by SQuIRe (Stroke Quality Improvement for Rehabilitation) establishing two new Stroke Recovery Services in Bradford and Doncaster. The Stroke Recovery Services support stroke survivors and their families from hospital to home using a person centred approach to address the practical and emotional needs of stroke.</p><p>We&rsquo;re looking for innovative, passionate and professional individuals with excellent communication, interpersonal and organisational skills to join the team in delivering this project.</p><p>Key responsibilities will include:</p><p><ul><li>Setting up and establishing new services working with external stakeholders to achieve this.</li><li>Working with stroke survivors and carers to identify their needs, providing person centred support which enables them to achieve their desired outcomes.</li><li>Delivering an effective service in line with our case management principles ensuring that confidential and accurate records are kept on our CRM database.</li><li>Supporting clients to make informed lifestyle changes which will help them to prevent further strokes.</li><li>Working collaboratively with other professionals and organisations involved in the care of stroke survivors and carers to ensure the best possible support is provided throughout the stroke pathway.</li><li>Contributing to the delivery and evaluation of the project.</li></ul></p><p><b>About You</b></p><p>The Stroke Support Coordinator will have:</p><p><ul><li>Experience of providing person centred support.</li><li>Experience of working with people with a disability or long term health condition, and their carers.</li><li>Experience of using technology and IT systems to support your work and keep timely, accurate records.</li><li>Effective listening skills with the ability to communicate clearly with a wide range of people.</li><li>Experience of working with health and social care professionals in a variety of settings.</li></ul></p><p>This role requires extensive travel across a large geographical locality including the delivery of face to face support in the form of hospital and home visits. Some time may also be spent working within a hospital team.
