Þpartment Post Acute Access Job Summary The Post-Acute Access Coordinator provides support to the Post-Acute Access department to ensure efficiency of the admissions process and that the successful transition of each new admission to SMH Post-Acute Services. Required Qualifications - Require current Florida licensure in a clinical professional healthcare field (i.e., LPN, LPTA, COTA). - Require a minimum of three (3) years of clinical experience in a hospital or post-acute setting. - Require Certified Healthcare Access Associate (CHAA) certification within one (1) year of hire. Preferred Qualifications - Prefer the ability to work cohesively with care team and interact with all levels of the organization. - Prefer the ability to seek new methods to incorporate into existing practice. - Prefer previous experience with payors, post-acute guidelines, and patient placement. - Prefer persuasive oral and written communication skills. - Prefer the ability to coordinate a multitude of tasks in a fast-paced environment, as well as, handling multiple tasks simultaneously. - Prefer the ability to handle stressful situations with tact and diplomacy. - Prefer the ability to make independent decisions when indicated, and shift priorities with flexibility.
